Foto Forum Santa Fe is an educational institution, and violation of this policy in our programs will not be tolerated on the part of the Instructor, Teaching Assistant, Intern, participant, or any third party. Harassment of any kind in Foto Forum Santa Fe programs may result in immediate removal from programs without refund of payments rendered. Conduct that will result in removal from from programs includes, but is not limited to, harassment based on age, race, color, creed, military status, national origin, religion, gender, sexual orientation, physical or mental disability, as well as sexual harassment, which includes, but is not limited to, unwelcome sexual advances, requests for sexual favors, and/or verbal or physical conduct of a sexual nature including but not limited to sexually-related drawings, pictures, jokes, teasing, e-mails, text messages, uninvited touching, or other sexually-related comments. The conduct prohibited by this policy includes conduct in any form including but not limited to e-mail, voicemail, chat rooms, Internet use or history, text messages, pictures, images, writings, words, or gestures.
if any participant believes they there has been a violation of these policies, they should notify the Instructor or Teaching Assistant of their workshop immediately.

I understand that I will be working with photo chemicals and will follow the reasonable direction of the instructor and/or adult volunteer.

I agree that I will be responsible for all medical expenses relating to an injury as a result of participation in the class work at Foto Forum Santa Fe, whether or not caused by Foto Forum its employees, volunteers, agents or other participants.

I understand that in the participation in the class work or photo shoot at Foto Forum Santa Fe there is risk of injury to the body, psyche or property damage to myself and others. Such injuries may be caused by, but is not limited to, other persons, fellow students accidentally, intentionally, self-inflicted or by faulty equipment or facilities, or by vehicle accidents while in transit to or from Foto Forum or through the activities themselves.
RELEASE AND WAIVER OF LIABILITY
I agree on behalf of myself and my heirs, successors and assigns, to release and waive any and all claims for damages which I may have and to release and discharge in advance Foto Form Santa Fe and its Officers Directors , employees, agents and/or volunteers from any and all claims for damages I may have and I further agree to indemnify and hold harmless advance Foto Form and its Officers Directors, employees, agents and or volunteers for claims of damages arising out of my participation in the class work at Foto Forum.
